Palliative care services are provided by two distinct categories of health and social care professionals:

those providing the day-to-day care to patients and carers in their homes and in hospitals, and
-
those who specialise in palliative care (consultants in palliative medicine and clinical nurse specialists in palliative care, for example).

GPs
A majority of GP practices and community nurses hold a palliative care meeting monthly. GPs work closely with other services to provide medical care and advice for patients and their carers. GPs provide medical, psychological and social support for palliative care patients and their carers on a daily basis. This will involve liaising with other services and professionals. Tower Hamlets Palliative Care Centre aims to provide and promote this coordinated care.
District Nursing Service
The adult community nursing service is a 24 hour service provided 365 days per year.
The service includes district nurses, community matrons, the community liaison service and case managers. The teams work closely together to provide a high quality integrated service with the aim of meeting your health and well-being needs by offering optimal support to maintain you in your own home where appropriate.
The team is able to provide skilled nursing care, advice on social support, and help to manage your long term conditions.
The service is based in four locations which are aligned to the primary care networks and social services.

St Joseph’s Hospice
St Joseph’s Hospice is one of the largest hospices in the UK, set in the heart of east London.
The Hospice offers comprehensive palliative care services for people with progressive and life threatening conditions – including inpatient, day hospice, out patient and home based care. They work to meet all the needs of their patients, including those which reflect their ethnic, cultural or religious heritage.
Home based care, day hospice, outpatient and inpatient services are available to people registered with a GP in City and Hackney, Tower Hamlets and Newham.
All services are provided free. St Joseph’s is a charity and depends on the generosity of its supporters to enable it to continue its work.

The Carer’s Centre – Care Plus Service
The Princess Royal Trust for Carers was created on the initiative of HRH The Princess Royal in 1991. The Carer’s Centre provides support for carers including information, advice and support services. The Trust also acts independently in the interests of carers through:

Macmillan End Of Life Occupational Therapy Service
Macmillan End Of Life Occupational Therapy (OT) Service aims to provide a specialist OT community service to clients with complex palliative conditions, age 16 or over, who live in Tower Hamlets.
Clients are usually, although not always, in need of specialist palliative care services from the community palliative care team (CPCT), and the service is closely linked with the CPCT, working with the clinical nurse specialists and social workers within the team.
Occupational therapy focuses on the person, their physical, social and psychological situation, occupations and lifestyle, and functional ability. The service aims to develop achievable goals to maximise and maintain a person’s independence for as long as possible.

Psychology Service
The psychology and counselling service offers local people the opportunity to discuss their worries and stresses with a psychologist or counsellor in a professional setting. If you are not comfortable talking in English, the team will find a health advocate who speaks your language. There is a service for people who use British Sign Language. There are counsellors who speak Bengali, Sylheti, Somali and some other languages.

Marie Curie nurses
Referrals are made by community nurses and they can provide a night /day nurse to stay for a period of time with the patient if there is an identified nursing need.
Contact the local community nurse or the Tower Hamlets Palliative Care Centre (see above) who can make the referral.
Age Concern advancing illness and end of life care support network
This service is for patients who are palliative and can offer up to 14 hours support per week to older people (60+) who are isolated due to limited informal care network, as well as befriending, signposting to other services, and practical support.
Barts and The London NHS Trust/MacMillan Specialist Palliative Care Team
The team includes senior clinical nurse specialists, doctors who are trained in palliative medicine and junior doctors in training. They have further support from a consultant clinical psychologist, social workers and the chaplaincy. They also have access to the full range of multi-disciplinary services including occupational therapy, physiotherapy, dietetics and further oncological advice.
