The district nursing team is part of the community nursing service, and provides a service for adults requiring nursing care in the community.
An adult can have contact with the district nurse for a variety of reasons including:

simple and complex wound care assessment and treatment,

palliative care assessment, support and symptom control,

continence assessment,
-
continuing care assessment,
-
case management of patients with long-term conditions such as diabetes and coronary heart disease,
-
administration of intravenous therapy as prescribed and care for patients with cytotoxic IV drug therapy, and
-
health promotion/lifestyle advice.
The district nursing service provides nursing care, case management, support and advice in the homes of patients who have difficulty with mobility outside of the home. This enables better patient choice and provides specialist nursing care that cannot be provided by the practice nurse.
How to get an appointment

The service will be provided to any Tower Hamlets resident over 18 requiring nursing care in the home or specialist care that can only be delivered by the district nursing team.
Referral method
Monday to Friday, from 9am to 5pm, referrals should be made via the adult health team. Please telephone 020 8223 8871 or 020 8223 8872.
Monday to Friday after 5pm, all weekend and on bank holidays, referrals should be made via the GP out-of-hours service. Please telephone 020 7377 7151 or 020 7377 7201.
Anyone can refer into the service, and it will be a 24 hour service by 2009.
