We provide an inter-disciplinary rehabilitation service to improve the health, independence, and lives of adults aged 16-64 with severe complex physical disability, living in the community. The disability options team is part of the older people and rehabilitation service.
What services are provided?
We provide specialist assessment, therapy and treatment for adults with neuro-disability and physical disability. Community care assessments and packages are offered by social workers linked to the team.
Where is the service provided?
Clients are seen wherever it is most appropriate for them. This may be at home, at work, at college or another community setting, or it may be in one of the hospital outpatient departments or clinics.
What is the referral process?
Most referrals are by health professionals and social services staff, but anyone can refer to the team, including the client and non-health professionals.
