Diabetes service

The diabetes team at Tower Hamlets Primary Care Trust provides a wide range of services, including: 

 

  • one-to-one consultations for individuals, families and carers at the Diabetes Centre and at some GP surgeries,

  • education for type 2 diabetes at the Diabetes Centre and in the community, known as HAMLET,

  • education for type 1 diabetes at the Diabetes Centre, known as DIANA,

  • education and training on appropriate diet for diabetes to members of the Diabetes Team and staff involved in the care of diabetic patients within Tower Hamlets PCT,

  • education/information on diabetes/complications/diet for patients,

  • support in the transition from diet to tablets and then tablets to insulin for patients/carers and staff,

  • referral to other members of the team as required for example – dietitian or podiatrist,

  • small group education sessions on diabetes/ insulin as required,

  • retinal screening,

  • glucose tolerance testing,

  • an insulin pump service.


Where to find us

 

The Diabetes Centre is open from 8.30am-5pm Monday to Friday. Consultations are by appointment only – we do not offer a drop in service but will always try to help solve any problems.

 

One-to-one appointments are also available at some GP surgeries (usually either one morning or one afternoon per month).

 

Structured education at Mile End Hospital (within the education programme) and certain GP surgeries. One-off programmes are held by prior arrangement with selected surgeries.

 

How to get an appointment

 

Generally your GP should refer you if they consider you need to see the consultant, diabetes nurse or to attend one of the education sessions. Your GP practice will have the required referral forms and will know how to contact us.

 

If you have not had your annual diabetes eye screening you should either contact your GP or the service directly on 020 8223 8625/8626.
