The primary and community care commissioning team aims to improve health and well-being by commissioning healthcare for local people and a range of community health services.
By working in partnership with organisations and listening to the communities we serve, we aim to provide the best possible healthcare and advice for those living and working in Tower Hamlets.
We have developed innovative and collaborative frameworks for commissioning and performance management of our primary care contractors and provider services.
We are responsible for using 'World class commissioning competencies' (measurements of success) to secure high quality integrated services, in line with local health needs and delivering service improvement and redesign in healthcare.
Our goals
We have a number of goals for 2008/2009, including:

implementing shifts of care from hospital to community settings,

implementing strategies, eg access to local care and services, improving health and well-being, oral health, urgent care, pharmacy services, long-term conditions management, and sexual health,

promoting safe, effective and cost-effective use of medicines across Tower Hamlets,

enabling diversity and equality issues to be visible and valued,
-
ensuring effective public and patient involvement and scrutiny of our work through local area partnerships, the overview and scrutiny committee, LINKS and patient group attendance,

supporting the development and implementation of the joint NHS Tower Hamlets and London Borough of Tower Hamlets improving health and well-being strategy.
